What type of laser do we have in our clinic? 
The infra-red 1064 ND: YAG. "The Lyra i Laser System" made by Laserscope Co. This machine has been FDA approved and is less dependent on skin color than the Ruby or Alexandrite machines. The wave length is safe for all skin colors. This laser can be used for both light and dark skinned patients and comes with a full contact cooling device (tip) to help with the discomfort of the procedure.

 

Spider Vein Reduction Information:

This is a revolutionary new laser treatment that can quickly and safely eliminate unsightly leg veins, spider veins & broken capillaries up to 4 mm in diameter.  Unsightly leg veins that result from heredity, pregnancy, trauma and the normal aging process are NOT necessary to circulatory system and can be removed without creating a health problem.    Our advanced technology minimizes the redness and bruising normally associated with traditional treatments.  Best of all, our technology allows us to treat people of any ethnic origin and skin type including tanned skin.

How does it work?

The laser system is designed to treat veins safely and effectively.  The laser light is absorbed by the pigment (color) in your blood vessel, generates heat, resulting in blood coagulation, which collapses the unwanted vessel wall.  The laser energy is delivered through a hand-held device with a cooling tip that protects your skin.  The Lyra Laserscope Laser we have in our clinic has revolutionized the removal of leg veins.  Studies show a 50-75% clearance rate with very little discomfort and no bruising. 

When will you see results & how long do they last?

The appearance of leg veins will either dramatically fade or completely disappear during the treatment.  Some vessels may show temporary swelling that will subside after a few hours to a few days.  The redness of the treated vessels will gradually disappear as the healing process occurs.  This may take between two and eight weeks.  The factors that caused your vessels to begin with may still exist (for instance, if you have a  job that you are on your feet all day)......may be the reason for recurrence in the future.  It would never be the same vessels, however, it would be in another area where the blood could force itself out to form other vessels.  You can have continued clearing of the vessels for up to 5 months following treatment.  Facial veins (nose, cheeks, etc.) usually resolve within 2 weeks, however leg veins take 8-12 weeks.  At the end of your 1st treatment, some vessels may be gone while others may be lighter or seem not affected.   Realistically, a series of 2-3 laser treatments at 6-8 week intervals, is needed to achieve total clearance.  Any sooner doesn't give the body time to absorb the treatment already done.  Any hyperpigmentation, if any, to the skin will be absent by your next treatment.   MOST PATIENTS SEE EXCELLENT IMPROVEMENT AFTER JUST TWO TREATMENTS.

Are there any side effects or risks?

Immediately following the procedure you may experience some redness and a small amount of discomfort in the treated area.  The redness usually dissipates within one to three days.  You would have a definite cat-scratch effect after the procedure.   Any hive formations usually leave by the next day.  Blister/hive formation, vessel swelling, pigmentary changes, "cold sore like" blistering, are all potentially possible, however, very very rare.  Also, even though it is not the intended purpose of this particular treatment, hair growth may be impaired in the treated area.

What other methods help veins?

Until laser treatment was introduced, sclerotherapy has been the only small vein removal system.  Sclerotherapy is the needle injection of an irritant directly into the leg vein, followed by several weeks of swelling and irritation.  This procedure is time consuming, painful, and difficult to tolerate.  There is a some risk of hyperpigmentation and ulcers.  At your consultation, we will determine if your veins are appropriate for laser treatment.  If not, we will refer you to a clinic  who does sclerotherapy.  There are new methods of removing the larger varicose veins by Cosmetic Surgery offices as well, that we could refer you to. 

What do you have to do before Treatment of the Veins?

  • Shave the area to be treated within 12 hours prior to treatment
  • Do not take blood thinning medications and anti-inflammatories for 2 weeks prior to treatment (Cumidin, Aspirin, Motrin, Anaprox, Aleve, Advil, Ibuprophen)
  • Do not use ginko and St. John's wart for 10 days prior to treatment
  • Do not use tetracyclines and Zithromax for 7 days prior to treatment
  • Do not wear mascara the morning of your treatment if treatment is on the face (because of the eye protection goggles) may cause blurriness after treatment
  • If you purchase anesthetic topical cream, apply it 1 hour before treatment.
  • Apply SPF 30 sun block on just treated area if you will be exposed to sun 3 weeks before treatment

What do you have to do after Treatment of the Veins so I can plan for this?

  1. No exercise for 3 days at least (excercise will increase blood pressure and flow).  It is recommended by the LaserScope Company that you do your part by resting as much as you can off your feet for the next few days.
  2. Do not use alcohol or take the blood thinners mentioned above for 5 days after treatment
  3. Avoid sun exposure and wear SPF 30 sunblock for 3-4 weeks after treatment on just treated areas
  4. Continuously wear support hose (you can purchase them at Salmons Pharmacy 450 W. 800 No. Orem - in the Pinehurst Plaza behind the Sonic Drive-In Restaurant)  for 3-5 days if treatment area was moderate to extensive
  5. Apply antibacterial ointment (over the counter Neosporin) 3 times a day for 7 days
  6. Apply cold compresses if redness, swelling or hiving occur
  7. Leave the pressure bandage on that we put on you for 24-48 hours.......then switch to above support hose
  8. Do NOT take HOT shower/bath, hot tubs, saunas for 3-5 days
  9. Keep treatment area clean by gently washing it with tepid water and mild soap

Patients Who Have Signed Up for Laser Vein Reduction

We want you to be prepared with medical stockings ready after your vein reduction, therefore we are giving you this information to pursue before your appointment.  Compression stockings provide support and motion to the legs.  The veins and muscles in the legs are particularly susceptible to circulatory disorders because they are far from, and below, the heart - from that point, returning blood to the heart is an "uphill" job.  Additionally, the occupations and lifestyle habits common to industrialized societies can, over time, compromise these very important components of our circulatory system, causing leg fatigue, pain, and unpleasant varicose veins.  The cornerstone in the treatment of acute and chronic poor circulation in the legs is compression therapy.  The goals of compression therapy are relief from unsightly symptoms and prevention of uncomfortable swelling.

The application of elastic compression stockings is a medically effective method of relieving leg discomfort due to poor circulation.  The highest amount of support is applied at the ankle and then is gradually reduced along the length of the stocking.  This provides a massaging effect that enables the veins to perform their function more efficiently - circulation improves, relief follows.

Salmon's Pharmacy carries a very good brand and we recommend you purchase some.  If you want to use Ace Bandages, that is fine as well, they are just not something that will stay if you are walking around a lot or if you have many areas done at the same time.  We feel it is important therefore, the information is as follows...

Salmon's Pharmacy (450 West 800 North, Orem, in the Pinehurst Plaza behind the Sonic Drive In Restraurant) carry a medical strength of "Support Stocking."  Please go there and be measured for the "TruForm Medical Support Stocking."  The cost depends the type, either Thigh Hi ($34.89) or Knee Hi ($24.89).  Which type you buy will depend on whether you are having vein reduction on upper or lower leg.  They come in small, medium, large and extra large.  These don't require a prescription, however, the type you buy must be the 20-30 PSI compostition to help any swelling.

LASER HAIR REMOVAL INFORMATION:

 

Laser stands for Light Amplification Stimulated Emission of Radiation. The highly concentrated, yet non-ionized laser light amplification is transmitted 1-5 millimeters onto the treatment area. The laser targets the melanin (pigment) in hair follicles while the hair is in its active growth phase only. Laser pulses are then applied over the area. The follicle absorbs the light, where it is converted into heat, causing thermal disabling to the follicle base without harming the surrounding tissue. Hair regrowth is impaired. Only 30% of your hair is in the active phase at any one time. Hairs will visibly shed for up to 3 weeks after treatment.

Is laser hair reduction permanent? 

Though the FDA has approved the Laserscope laser for permanent hair reduction, results cannot be precisely predicted. The results are variable, even for the best lasers. It is impossible to guess how many sessions an individual will require. Current studies have shown even though a permanent reduction in hair growth occurs, some hair regrowth may occur over time, although this is usually much slower and much finer than before treatment occurred. True permanent total hair removal is possible with repeated recommended treatments. Lasers have only been out 12 years and therefore, permanency can only be predicted by that parameter.

Why are more than 1 session required? 

Although 1 session will produce long-term hair removal, multiple sessions are necessary due to the 3 phases of hair growth. Laser energy has its greatest thrust in the growth stage. After your first treatment, the dormant stage will then start in transition to become active. Usually 6-8 treatments will be necessary for satisfactory, long-lasting results. The laser company recommends that your 2nd session be scheduled 1 month after the 1st session. After that, treatments will depend on the body part as follows:

        Legs 3-4 months apart              Bikini line 6-8 weeks apart                Face 6 weeks apart                            All others 2-3 months apart

How long does the procedure take?

Each area has different parameters and reactions that must be determined by the laser operator on your first visit, which takes approximately 5-10 minutes. After the test area is done, the length of the procedure depends on the area. Upper lip and chin may take 15 minutes, where a man’s hairy back may take 1-1/2 hours. Men’s shoulders take about 20 minutes. Underarms 15-20 minutes, bikini line about 15-30 minutes. Legs 1-1/2 hours.

Does the procedure hurt? 

Because of the deeper penetration of our longer pulse, there is usually some discomfort. Some patients describe the sensation as a "pop" (similar to a rubber band snap), as the follicle absorbs the laser energy. Some feel it is similar to a tingling sensation. Discomfort is minimized by chilling the skin 3-4 minutes before and during treatment. Some patients say they don’t experience any discomfort and yet others say it is a mild sting. Anesthetic cream can be used before treatments in sensitive areas. To minimize discomfort, anesthetic cream can be included as part of a treatment package. Should you chose to use our cream, you will need to apply it to clean skin approximately 1 hour prior to your treatment time. After the treatment, the area may feel like a mild sunburn for a few hours.

Who are the best candidates for laser hair removal, dark/light hair, dark/light skin? 

Light skin and coarse dark hair are the best candidates. The darker the hair the more laser energy is absorbed. White and blonde hair contains pheomelanin pigment which absorbs laser energy less than the eumelanin pigment, present in black or brown hair. Therefore, white and blonde hair types may require more treatments. A Laser Company in California has patented a natural liposome cream that is successful to darken hair down to the follicle. It is called Meladine and is considered safe for all skin types. It will not color the skin; it will only absorb into the hair follicle. It temporarily provides a target for the laser beam and is making this procedure 95% effective. It needs to be started 14 days prior to the laser treatment, twice daily. We can make it available to you as part of your treatment package when you schedule your appointment at the check out desk.

What if I bleach my facial hair every couple of weeks? 

We would ask you to have no bleach on your hair 3 weeks prior to your laser treatment.

What if I tan my skin? 

Tanned skin robs laser energy as it passes through the skin to the hair follicle. Our laser is less dependent on skin color than other lasers. But, to help prevent melanin coming to the surface of the skin before laser treatment time, we ask that you not tan the area or use self-tanning creams for at least 3 weeks prior to treatment.

Is there any area that a laser can’t go? 

The glabela (between eyebrows) and the eye brow areas are somewhat precarious.  We have been trained to target the laser toward the bone of the eye-brow and with our special metal eye protection, we feel confident in doing this area.

How big an area can the laser get at any one time?

For laser hair removal the laser can be enlarged as much as 10 mm (about the diameter of a dime).

I have lip liner tatoo.........can you still do my lip? 

Our laser is meant to by-pass skin by going straight to the follicle 4-6 mm deep, however, there is a chance the melanin in a tatoo may be damaged if the laser gets too close. We will stay away from the tatoo to prevent any lightening. The same goes with brown spots on the face.

Do some areas respond faster than others? 

Yes, face, armpits and bikini lines respond fastest to laser treatments. These are also the most sensitive area to treat. The scalp is very resistant to treatment.

What if you take some medications or use creams (i.e. Retin A, Accutane)? 

We ask that you discontinue using Retin A or Glycols that are in some lotions 3-5 days before your laser treatment. If you are on Accutane, you have to wait 6 months before treatment. Hair removal creams (such as Nair), should also be avoided 3-5 days prior to treatment, if you are using them.

If you wax your legs or do electrolysis, when should I do laser hair reduction? 

Waxing legs or plucking hairs, removes the hair shaft from the hair follicle and decreases the effectiveness of the laser. Your last waxing, plucking or electrolosis session should have been at least 4 weeks ago for laser treatment to be effective.  We need adequate growth within the follicle.

After treatment, what do I expect? 

There is usually mild redness and occasional mild stinging, which goes away within 1-2 hours. A cool pack can be applied as well as hydrocortisone cream (i.e. Cortaid, which is over the counter at any pharmacy)  Some people describe post-treatment like a mild sunburn for a fwe hours.  Scarring does not occur at the energy levels used for hair removal, although patients with very dark skin may have some temporary lightening of skin color.

If I am pregnant or possibly pregnant, can I get the ANY laser treatment?

No, the laser company has informed us that as a company their stand is to not allow pregnant people to get laser treatment. We will abide by their recommendations as well, even though there is no exposure for a in-utero fetus. 

If I am breastfeeding, can I get the laser treatment?

Yes, however, because of hormone changes in the body, it is recommended to wait a few months after the birth of your child to have the laser treatment. Hormones do change the hair cycles in our bodies.

Is there any restriction on age for laser treatment?

No pediatric patients are approved for laser treatment. Therefore, as soon as women reach menarchial age, they would be eligible candidates for laser treatment.

Safety Issues:

The YAG laser does not leave any residual light in your body like x-rays (because of the non-ionizing light energy that doesn't disturb the DNA of the body), therefore, it is safe.  Our operators are extensively trained and have a protocol to follow by the company that will insure your safety. If you, as a patient, follow the instructions about pre-treatment sun exposure there can be no burning or blistering of the skin. The sun forces the melanin to rise to the skin’s surface (which is what causes us to turn red or tan). If there is melanin on the skin’s surface, the laser energy becomes defused out onto the skin as well as the follicle. This reduces the effectiveness of the laser to hit its strength to the hair follicle as well as risks the skin. It is important to following the sun exposure instructions carefully.  Patients have asked if children can come with them.  No, they can't be in the same room and we don't have baby-sitters on staff in the waiting room, so we ask you look out for their safety and not bring children.

What do you have to do before Hair Removal Treatment?

  • Apply SPF 30 sunblock to treatment areas that are exposed to the sun, tanning beds, or tanning lotions 4 weeks prior to treatment
  • Shave the area to be treated within 12 hours prior to treatment
  • Do not wax, tweeze or bleach your hair for 4 weeks prior to treatment
  • Do not use hair removal lotions (i.e. Nair) or products witih glycol in them 3 days prior to treatment
  • If doing your arm-pits, do not use deodorant the morning of treatment
  • Do not wear mascara the morning of treatment (because of eye protection glasses)
  • If treatment area is on face, wash your face that morning and apply no lotions

What do you have to do after Hair Removal Treatment so I can plan for this?

  • Apply SPF 30 sun block for 2 weeks after treatment and do not purposely expose treated areas to the sun
  • No plucking or waxing between treatments
  • Keep area clean while it's still tender (wash tepid water and mild soap)
  • No Retin-A or lotions with glycol or hair removal lotions (i.e. Nair) for 3 days post treatment
  • If swelling or "hot sensation" occurs, cold compresses can be applied to reduce redness and swelling
  • Apply 1% hydrocortisone cream (over the counter) in a thin layer after cold compresses to help reduce redness (only if necessary)
  • Avoid hot showers, sauna and swimming pools for 1 day after treatment

 

FACIAL SKIN REJUVENATION WITH THE LASER

HOW DOES IT WORK?

Laser light is absorbed by pigmented areas where it is converted to heat, causing a breakup in the pigment. In broken capillaries and spider veins, the laser energy is absorbed by the pigment in the blood and collapses the unwanted vein. For wrinkles and skin toning, the laser stimulates collagen growth.

When will you see results?

Effects are cumulative. After the first treatment you will see improvements to broken capillaries, small facial veins and some pigmented areas within a few days. Other improvements such as wrinkle reduction and toning will be more subtle and may take about three months. Friends may suggest that you look more rested. With each subsequent treatment, subtle changes will accumulate until you have achieved the improvement you desire.

How often can you have it done?

A series of three to six treatments, spaced three to four weeks apart, is our standard recommendation. Occasionally, more treatments may be needed for certain individuals or conditions. Periodic occasional maintenance treatment may be prescribed for optimal, lasting results.

What can you expect immediately post-treatment?

Immediately following the procedure, you may experience some redness, swelling and/or a slight darkening of pigmented areas. The redness and swelling usually dissipate within one to three days however, darkened pigmentation may persist and peel lightly for up to one week. Makeup can be applied to the treated area immediately, if necessary. Additionally, hair growth may be impaired in the treated area, although it is not the intended purpose of this particular treatment. It is extremely important to apply a moisturizer with SPF30 sunblock and avoid sun exposure on a daily basis to prevent additional sun damage.

WHAT TO DO BEFORE THE TREATMENT:

  • Do not tan for four weeks prior to treatment
  • Apply SPF 30 sunblock to treatment area 4 weeks prior to treatment
  • Discontinue all Retin-A, glycolic acids, vitamin C products, 3 days prior to treatment
  • Inform us if you have taken Accutane within past year or Tetracycline in last month
  • Inform us of any changes in your medical history since your last visit
  • Remove makeup, cleanse and completely dry the area to be treated prior to treatment

WHAT TO DO AFTER THE TREATMENT:

  • Cool compresses can be used, if needed, to decrease redness or swelling
  • Wash area twice a day with gentle cleanser, cool or tepid water and your hands only. Pat dry.
  • A moisturizer containing SPF 30 should be applied daily and reapplied during the day if necessary
  • Avoid unnecessary sun exposure
  • Use of Retin A, Renova, glycolic acids, vitamin C products, and any other potential irritants may be started after all irritation from the procedure resolves
  • Please call at the first sign of persistent pain or blistering
  • Do not exercise vigorously for 48 hours after treatment
  • Do not take hot showers,, baths, or use saunas for 48 hours after treatment.
  • Continue use of anti-viral medication if such as been used for cold sores

EXPECTATIONS AFTER TREATMENT:

  • Expect some redness and swelling that may last from a few hours to a few days
  • Expect peeling to occur on days three and four
  • 5-6 treatments may be needed for maximum effect, based on 3-4 weeks apart. After that, every 3-4 months to keep the collagen plumping effects.
  • Photo damage will re-occur if the patient does not avoid sun exposure and use sunblock of at least SPF30 on a daily basis. We carry Skinceuticals SunBlock, which is the best on the market (talk to your laser tech. at your consultation to learn more about it)

Shaving Bumps:

The US Military installations all have our Laserscope Lyra I machine in it because they found it to be most effective on shaving bumps for their military men. Shaving bumps are an incredible cost to the military in men who shave over them and get infections. The military found too many men spending days in the infirmary and not in service because of it and searched out what laser helped cure shaving bumps the best. Our laser will get rid of problems with shaving bumps because it debilitates the hair follicle to not grow, therefore, keeping the incidence of shaving bumps to a very minimum. Call for a consultation.


If you have any of the following conditions, please call our office to see the contra-indications before you make your laser appointment.

Pregnant - You cannot receive laser treatment during pregnancy

Hirsutism (Extra Growth of Hair) - If you have been diagnosed with this then the laser treatment may offer a temporary benefit but may need continued treatment.

Transplant Anti-Rejection Drugs - Use of these drugs may cause a change in your hair growth, therefore, laser treatment may offer a temporary benefit but may need continued treatment.

Lupus (or other auto-immune deficiency diseases) - you should not have laser treatment at this time.

Bleeding Abnormalities - you should not have laser treatment at this time

Accutane Treatment - you cannot have laser treatment within 6 months of taking Accutane

Kelloid or very thick scarring - you would be at elevated risk for scarring or for hyper or hypo-pigmentation of the skin.  you may not be a candidate for laser treament in that particular area only.

Psoriasis or Vitiligo - you should not have laser treament in those areas

Pulmonary embolism/blood clot - you cannot be treated with laser for leg veins

Chemical Peels, Micro-Dermabrasion, Laser Resurfacing or Face Lift - you cannot be treated with the laser immediately after these facial procedures.  You should wait approximately 1 month for rejuvenation of natural cells.

Leg ulcer or Phlebitis - you cannot be treated with the laser for leg veins

Blood thinning medication - you cannot be treated with the laser for leg veins

Lotions with Glycol - stop using 3 days prior to laser treatment because of excessive drying of the skin.

Rheumatoid Arthritis "Gold" Therapy - Obtain consent from your Rheumatologist before any laser treatment

Herpes Simplex or Fever Blisters - you cannot be in an active phase of herpes in the area you want treated by the laser.  If you have no current outbreak, it is recommended that you be on an anti-viral medication three days prior to laer treatment and three days after the treatment is done.  See your primary care provider for medication if you don't have any.

Diabetes - because diabetes sometimes causes impaired circulation or ulceration that can alter your healing properties, laser treatment is not recommended

Epilepsy (this is only the light sensitive epilepsy) - we must warn you that even with eye protection, you may sense a "flash" when treatments are done in the facial area

Dark Spots after pregnancy or skin injury - we must warn you that you would be at an elevated risk for scaring or for hyper or hypo-pigmentation.  The choice sould be yours as to whether you want to risk the laser treatment

HIV - if you have aids and are healthy you must obtain permission from your primary care physician to proceed with laser treatment

Waxing/Plucking/Electrolysis - you must wait 4-6 weeks before having the laser treatment because of the need to have an active-phase growth hair in the follicle we are targeting with the laser

Moles in the same area to be treated by laser - we will not put the laser beam on any mole until that mole has been looked at by one of our primary care nurse practitioners/midwives to make sure it is not cancerous.  We are able to treat the surrounding area.

Dipilatories (hair removal creams such as Nair): if you are using a dipilatory such as Nair on your hair before your appointment for hair removal, you must stop using it 3-4 days prior to your laser treatment because we want the hair follicle to grow to the surface of the skin so it can be the target for the laser, because of the melanin in the hair. Nair and other hair removal creams dissolve the hair 1/8th of an inch below the surface of the skin, therefore, if you do it 3-4 days prior to your treatment, the hair should be grown back to the surface by the time you come in. Most people shave the hair from the areas they want the hair removed but if that isn't possible or you have never shaved the area before, a dipilatory is a good method to have clean skin and yet the same effect as shaving if done 3-4 days prior to treatment.
